Sofosbuvir

Sofosbuvir is a counterfeit: the hepatitis C polymerase mistakes it for a uridine building block, incorporates it into the growing viral genome, and then finds the chain cannot be extended, so replication stops. One tablet a day turned an infection people used to carry for life into a curable one: 90% of previously untreated patients in the registration trial NEUTRINO had no detectable virus twelve weeks after finishing treatment, and 95% in genotype 2 in FISSION. The US Food and Drug Administration approved it in 2013, and it is now the backbone of several combination regimens. It is a small molecule rather than a peptide, held in this corpus for reference.

Mechanism

A prodrug metabolised to a uridine analogue triphosphate that the HCV NS5B polymerase incorporates, terminating the viral RNA chain.
