Dulaglutide

Dulaglutide is a copy of GLP-1 (glucagon-like peptide-1), the gut hormone released after eating, bolted onto a fragment of an antibody so the body clears it at antibody speed: one injection a week, against the few minutes the natural hormone survives. Sold as Trulicity, it has been FDA-approved for type 2 diabetes since 2014. Its central trial, REWIND, is the one in this class that mostly enrolled people who had not yet had a cardiovascular event, and it met superiority anyway. Across 9,901 adults at 371 sites followed a median of 5.4 years, heart attack, stroke and cardiovascular death occurred in 12.0 percent against 13.4 on placebo, a hazard ratio of 0.88.

Mechanism

A GLP-1 analogue fused to an IgG4 Fc fragment; activates GLP-1 receptors to raise glucose-dependent insulin secretion and slow gastric emptying.

Regulatory status

Approved by the FDA. Marketed as TRULICITY. Earliest approval 2014-09-18. Application BLA125469.

Cardiovascular events and mortality — REWIND — Dulaglutide 1.5 mg weekly vs Placebo, Participants with the event (%) by outcome — Data: Gerstein et al. 2019, Lancet (REWIND cardiovascular outcomes) · n=9901 with type 2 diabetes, median follow-up 5.4 years. Primary composite is nonfatal myocardial infarction, nonfatal stroke or cardiovascular death (594 vs 663 events; hazard ratio 0.88). All-cause mortality did not reach significance (p=0.067). Gastrointestinal adverse events were 47.4% vs 34.1% and are not plotted, because their scale would hide the event differences.
